• 2 Application Forms, completed and signed
• Completed Cover Page and Credit Card Authorization forms (print from browser)
• 2 Recent Passport-Type Photographs
• Letter from sending U.S. Government department or agency
  
  The letter must explain all of the following:
• Name of Applicant and Title/Occupation
• Purpose of travel
• Dates of travel
