REQUIREMENTS FOR U.S. CITIZENS TO OBTAIN VISA TO ENTER EQUADOR

Tourist or Business Visa (Valid up to six months)

• Submit valid, signed U.S. passport/must have at least 6 months remaining, after completion of stay in Equador
• Copy of round trip airline tickets or itinerary
• 2 Passport-type photograph
• Completed Cover Page and Credit Card Authorization forms (print from browser)
• Bank statement or letter of economic reponsibilty from applicant's company
The Bank Statement should show that you have an average of $6,000.00 monthly in your account. (Applicants under 16 do not need a bank account statement. They require a letter of economic responsibilty from their parents)

• Medical certificate (The certificate must state that you have no contagious deseases.
• A recent police record
• Letter from applicant's parents stating that they allow their child to leave the country and travel to Equador. (NOTE: Only if applicant is under 18)
  

TDS Service Fee:        $40.00 per visa
Consular Fee:             $30.00 per visa
Consular IX Fee:         $200.00

• Recent Bank statement
