Passport
Submit your most recently issued passport.
Photo
Photographs: Two recent 2" x 2" full-faced color photographs with a light, plain background and taken less than 6 months ago. The face must be square to the camera with the mouth closed (teeth cannot be shown). Photos showing the applicant even slightly smiling are not accepted. Click here to download photo guidelines.
Countersign the Photo: One of the photographs must be countersigned by the person who has also countersigned the application. Endorse the back of the photograph by writing "I certify that this is a true likeness of (insert the name of applicant)" and add your signature and the date on which the likeness was compared.
Applicant's relatives and notary public will not be accepted as countersignatory.
Letter of Authorization
Authorization Letter: Please click here and print THREE (3) copies of the authorization letter required for TDS to submit and collect your passport on your behalf.
Name Change
Change of Name? If you have changed your name since the issuance of the last passport, provide the original court order, marriage certificate, and/or divorce decree.
Proof of legal Status
Proof of legal status in USA: Copy of US permanent resident card, Valid U.S. visa, or US passport.

Additional Helpful Information
Your application will be hand carried to the U.K. Embassy. There, the Passport Officer will examine the application and if all documents are in order, assign a date the passport will be issued (according to your departure date from U.S.). Processing time varies according to the departure date from the U.S., but is usually 6-8 weeks
If your passport has been lost / stolen or is unavailable you must provide the address at which you were living when the passport was first issued, the next of kin details included on your previous passport application form and the name of the person who countersigned the original application. For additional identification purposes please send your US passport or an enlarged clear copy (front and back) of your US Alien Registration Card and driving license
