Please send TDS your current valid and signed passport, plus one photocopy of the vital information page. It must not expire within 6 months and must have at least one blank visa page. If your passport expires within 6 months, you must renew your passport and then we will secure your visa with the new passport.

Photocopy of Alien Registration Card (Green Card) -or- evidence of legal status in USA (valid visa plus I-94 in passport)
In some cases the visa officer may ask to see the Original Green Card.
Proof of Departure
Please send a copy of your round trip airline tickets, itinerary or e-ticket, Cruise Itinerary or letter of confirmation from a Travel Agency confirming entry and exit from mainland China. The itinerary must show the applicant's name, date of arrival and departure from the country, flight or cruise information.

If Staying in a Hotel
If you are staying in a hotel(s) provide copies of confirmed hotel confirmations. Multiple hotels will require multiple confirmations covering your entire stay in China.
Or a letter of invitation from a Chinese tour operator.

If Staying with a Visiting Friend or Relative
If staying with a visiting friend or relative provide a signed letter of invitation from your host in China, must also include a copy of the host's Chinese ID
If you are a spouse or sibling traveling with someone on a business visa, the Host Company in China can provide a letter of invitation on your behalf.

Note:Application form MUST be typed, hand written applications will NOT be accepted. All questions MUST be answered, do not leave any question blank, type N/A if need be.
Note: All applicants must sign the application form even if the form is completed by another person, except that a parent or legal guardian may sign on behalf of an applicant under the age of 18.
Note: Applicants who complete the New York Application form MUST submit the form to the New York Office, it cannot be submitted to any other TDS Office.
Special Attention: Question 1.5 must be answered, you MUST retype your full name in your birth language. If your birth language is english you MUST retype your full name in english. If you birth language was something other than english you MUST retype your full name in your language at birth. N/A will NOT be accepted as an answer. Failure to correctly answer Question 1.5 WILL delay your visa application.
TDS Order Form
You must complete the Online Order Form.
The order form gives TDS your name, address, shipping information and payment method. Multiple applicants can be entered using one order form. You only need to complete one TDS order form per request. Please print two copies of the confirmation page. Include one copy with the documents you ship to TDS and keep one copy for your records.
While completing the online order form in this step, you will be asked how you want to pay TDS fees. You may pay our fees online with a credit card or enclose a personal or business check or money order.

Additional Helpful Information (Visa Notes)
Validity of The China Tourist Visa is usually valid for three months from date of issue, for length of stay not to exceed 30 days. Applications for a tourist stay exceeding 30-90 days are not usually granted, except from persons of Chinese origin.
