Please send TDS your current valid and signed passport. It must be valid for no less than 1 month beyond the date of your planned departure from Azerbaijan and must have at least one blank visa page.   If your passport expires within 1 month, you must renew your passport and then we will secure your visa with the new passport.

Please send 2 recent professional passport-type photographs.
Digital and non-passport photographs are not acceptable.

Photocopy of Alien Registration Card (Green Card) -or- evidence of legal status in USA (valid visa plus I-94 in passport)

Please send a copy of your round trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.

For Business and humanitarian visits the Invitation Letter from receiving party along with the copy of the charter and state registration certificate of the company/organization.

An official tourist voucher obtained from the travel agency or hotel which is approved by the Consular Department of the Ministry of Foreign Affairs of Azerbaijan

For visitor visas: An Invitation Letter from Azerbaijani citizen or foreign citizen currently residing in Azerbaijan along with their Azerbaijani ID.

Complete and sign One application. The Application Form must be typed.

A unique valid email address is needed for each visa application completed.

The "Humanity validation" (CAPTCHA) may require multiple attempts. Please be patient and refresh as many times as needed to pass the validation.

There is no clear space for a signature. After completing and printing the form, applicants must sign at the bottom of the print out.

You must complete the Online Order Form.

The order form gives TDS your name, address, shipping information and payment method. Multiple applicants can be entered using one order form. You only need to complete one TDS order form per request. Please print two copies of the confirmation page. Include one copy with the documents you ship to TDS and keep one copy for your records.

While completing the online order form in this step, you will be asked how you want to pay TDS fees. You may pay our fees online with a credit card or enclose a personal or business check or money order.

Urgent applications should be shipped to TDS using only the FedEx Company.
Please use FedEx Priority Overnight. Do NOT ship "First Overnight." This will delay the delivery of your package.

Visa Requirements, processing times, validity of the visa, and Embassy visa fees are subject to change without prior notice.

The Embassy of the Republic of Azerbaijan advises that the laws of the Republic of Azerbaijan prohibit visiting the occupied territories of the Nagorno-Karabakh region and seven adjacent districts without the explicit consent of the Azerbaijani authorities. It is illegal to travel to these regions without a special permit and visa issued by the authorities of the Republic of Azerbaijan.
