VISA APPLICATION FORMS

This Visa Application is on one page of white paper measuring 8 1/2 by 11 (letter size) or A4. It should not be enlarged or reduced. This application form may be used at Consulates General of Japan in the USA.

1. To print the visa application in Portable Document Format (PDF) you will require Adobe Acrobat Reader Version 3.0 or higher, free from Adobe Systems, Inc. You can download the Acrobat Reader version that is suitable to your system from the Adobe web site.
