- Personal history (resume) as a performer (in duplicate)
- Letter of invitation from promoter in Japan (in duplicate)
- Employment contract (2 copies)
- 2 stage photographs

- Guarantee letter from promoter in Japan (2 copies)
- Company registration of promoter in Japan (2 copies)
- Permission issued by bank of Japan (2 copies)
- Applicant's union card (if applicable)
- Acceptance contract between promoter and facility
- Certificate of tax payment of entertainment facility
- Details of entertainment facility (size and capacity)
NOTE
- Some applications may be referred to the Japanese government
for approval and would take 6 to 8 weeks to process, however
most applications are processed within 24 to 48 hours.
