• Submit your valid passport
• Completed Cover Page and Credit Card Authorization forms (print from browser)
• Affidavit of support from parent/spouse stationed in Japan      guaranteeing all responsibility for the applicant including      financial support
• Letter from the commanding officer in Japan confirming what accommodation arrangements have been made
• For child under 18 years, notarized letter of consent from     parent in usa required, if applicable
