4. The correct documentation required for a name change by marriage or court order. All documents submitted for a name change or a data correction must be the official seal of the issuing authority (see page two of the application form).

5. Completed Cover Page and Credit Card Authorization forms (print using the Print button on your browser)

6. Letter of Authorization. Click here to print the Authorization Letter.

You must print, then complete and include TWO COPIES of a Letter "Attn: U.S Passport Agency" stating: "I, (applicant name), hereby authorize Travel Document Systems to pick up my passport, and to discuss the status of my application with the Passport Services staff." The letter should be signed in BLUE ink.

7. Copy of itinerary of air tickets showing departure date from the U.S. or letter from employer stating immediate international travel is required

8. Appointment:

If you require an emergency passport issued in less than 1-3 days, you must have an appointment to proceed. Please call 1-800-874-5100 for an appointment. TDS must receive your documents at least one business prior to your appointment.

Your passport application is considered "expired" by the Passport Agency after 5 business days. Please please send your application to TDS the same day it is signed.

Fees:
There is no charge for the reapplication of a passport issued less than one year ago except when expediting is requested (issued in less than six weeks)

Include credit card authorization, a check or money order payable to "Travel Document Systems, Inc." for TDS Handling Fee, Consular Fee, and return FedEx.

Processing Information

Your application will be hand carried to the Passport Agency in Washington DC. There, the Passport Agent will examine the application and if all documents are in order, assign a date the passport will be issued, according to departure date from the U.S. Passport applications submitted by mail to passport agencies routinely take 6 to 6 weeks to be processed.
Urgent issue applications which may be issued between 1-3 business days require proof of departure within 7 business days of being submitted to the passport agency and MUST be shipped to TDS using only the FEDEX Company.
Other urgent applications can be issued as fast as 10+ business days .
TDS expedites applications by hand carrying them to the U.S. Passport Agency, and following up on the progress. Please note that TDS is not in a position to be able to offer any guarantees of passports to be issued by a specific date. Issue is determined by urgency of your departure from the U.S.
