Cover Page and Credit Card Authorization forms (print from browser)
- Letter from
sending Government Department or Agency, addressed to:
Embassy of Venezuela, Visa Section, Washington D. C.
The letter must explain all of the following:
- Applicant's
name and occupation
- Purpose of
the trip
- Dates of
the trip
- Contact person
in Venezuela
