These requirements are applicable for US Citizens applying for an Afghanistan Tourist visa in Los Angeles

PassportPassport

Please send TDS your current valid and signed passport. U.S passport must be valid 6 months beyond intended stay and must have three consecutive blank passport pages. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.
A visa cannot be stamped on a page even if only one Small Stamp is on the visa page If you do not have any blank visa pages in your passport, TDS can assist you in applying for additional pages. If your passport is not valid 6 months beyond intended stay, you must renew your passport and then we will secure your visa with the new passport.

Please provide a color copy of the photo page of your passport and a copy of your last valid Afghanistan visa, if your last valid Afghanistan visa is in an expired passport then please send a copy of the photo page and the page with your last valid visa. If you cannot produce those copies due to loss or theft then make a statement stating this and have it notarized when your application form is being notarized.

Please send a copy of your round-trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.

Invitation LetterInvitation Letter

If the applicant is being invited by a person or a reliable official or duly registered entity in Afghanistan an Invitation Letter is required. A letter form the Ministry of Foreign Affairs in Kabul authorizing the visit is also acceptable.

Provide a health clearance letter indicating that the applicant is not suffering from any contagious diseases especially Ebola.

Applicant must provide evidence of employment in the United States.

Additional DocumentAdditional Document

A letter addressed to the Consulate General of Afghanistan detailing the purpose of your visit, where and with whom the applicant will be residing, the residence address and details of your intended activities in Afghanistan during your visit. The letter should also indicate that the applicant is aware of the risks involved in the journey and takes full responsibility of any liability arising during or as a result of the trip to Afghanistan.
