Please send TDS your current valid and signed passport. U.S passport must be valid 6 months beyond intended stay and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.

Please send a copy of your round-trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.

You must submit a copy of the work permit approval issued to you by the Ministry of Foreign Affairs in Kabul.
If the work permit approval states, you can be issued a 1-year multiple entry visa both your business letter and letter of invitation must also request a 1-year multiple entry visa.

A business letter of responsibility (sample) from the sending company is required, to be typewritten on your company letterhead and addressed to:

The letter must explain all of the following:
Briefly introduce the applicant, include the position in the company and employment status of the applicant
Nature of business to be performed
Names and addresses of company(s) to be visited
This letter should request Multiple Entries visa if required
Guarantee of return transportation and sufficient funds for the visit
Applicants passport number
The letter should be signed by an officer of the company other than the applicant.

A business letter of invitation from the sponsoring company in Afghanistan is required, to be typewritten on your company letterhead and addressed to:
Consulate General of Afghanistan, Visa Section, Los Angeles CA
The letter must explain all of the following:
Briefly introduce the applicant, include the position in the company and employment status of the applicant
Nature of business to be performed
Names and addresses of company(s) to be visited
This letter should request Multiple Entries visa if required
The letter should be signed by an officer of the company other than the applicant.

Please provide a color copy of the photo page of your passport and a copy of your last valid Afghanistan visa, if your last valid Afghanistan visa is in an expired passport then please send a copy of the photo page and the page with your last valid visa. If you cannot produce those copies due to loss or theft then make a statement stating this and have it notarized when your application form is being notarized.
