These requirements are applicable for US Citizens applying for a Philippines Tourist visa in Chicago

The Consulate General of the  Philippines in Chicago, IL accepts visa applications for residents of the following states, Territories, and Countries:

Consular Jurisdiction: Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Nebraska, North Dakota, Ohio, South Dakota, Wisconsin

If you do not reside in one of the above locations please select a different TDS office.

Please send TDS your current valid and signed passport. It must not expire within 6 months and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.
A visa cannot be stamped on a page even if only one Small Stamp is on the visa page. If you do not have any blank visa pages in your passport, TDS can assist you in applying for additional pages. If your passport expires within 6 months, you must renew your passport and then we will secure your visa with the new passport.

Please send a copy of your round trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.
A photocopy of latest bank statement, previous year's income tax return, most recent pay stub and an employment certificate from the employer indicating position and salary, or affidavit of support. It is advisable to erase account numbers.
You must provide copies of all previous Philippines visas if you are applying for a multiple entry visa.

Note: For a minor traveling alone, or with the person other than his/her parent: Affidavit of consent and
support signed by at least one parent.

Former Filipino nationals can travel to the Philippines without a visa for a stay of up to one year. Their immediate family (spouse and children) also benefit from this visa waiver when travelling together.

Former Filipino nationals will be required to provide either a former Philippine passport or a copy of birth certificate on entry. Each accompanying family member will be required to carry documents showing proof of relationship:

A copy of marriage certificate for the spouse

A copy of birth certificate or adoption certificate for each child

The Philippines visa is usually issued for a single entry within 90 days of issue date, for a period of stay up to 59 days. Applications for extension of stay may be filed with local authorities after arrival.

Passport must have at least six months remaining validity and contain at least one blank visa page for visa to be issued.
