VISA APPLICATION FORMS
This Visa Application is on four
pages of white paper measuring 8 1/2 by 11 (letter size) which
must be photocopied back-to-back on a single sheet. It should
not be enlarged or reduced. This application form should only
be used at the Washington embassy.
