VISA APPLICATION FORMS
The
Tourist Visa Application is on four pages of white paper measuring
8 1/2 by 11 (letter size). It should not be enlarged or reduced.
The Business Visa Application is on three pages of white paper
measuring 8 1/2 by 11 (letter size). It should not be enlarged
or reduced.
These application forms should only be used at the Washington
embassy.

print the visa application in Portable Document Format (PDF) you
will require Adobe Acrobat Reader Version 3.0 or higher, free
from Adobe Systems, Inc. You can download the Acrobat Reader version
that is suitable to your system from the Adobe web site.
