U.S. Passport Application Forms

These Passport Applications are usually on two pages of white paper measuring 8 1/2 by 11 (letter size). The application should not be enlarged or reduced. Applications are also available from many Post Offices and County Courts.

1. To print these Passport Applications in Portable Document Format (PDF) you will require Adobe Acrobat Reader Version 3.0 or higher, free from Adobe Systems, Inc. You can download the Acrobat Reader version that is suitable to your system from the Adobe web site, and then return to this page.

DS-11 - First time passport issue
DS-11 - Renewing expired passport issued more than 15 years ago
DS-11 - Renewing expired passport for minor issued prior to age 16
DS-11 - For renewing mutilated passport
DS-11 - For replacing lost or stolen passport
DS-64 - Also required for replacing lost or stolen passport
DS-82 - Renewing expired passport issued less than 15 years ago
DS-82 - Applying for a second valid passport
DS-5504 - For passport re-application for amendment such as change of name
DS-4085 - For adding additional passport pages
