This Visa Application is on one page of white paper measuring 8 1/2 by 14 (LEGAL SIZE). It should not be enlarged or reduced. This application form should only be used at the Washington embassy.

1. To print the visa application in Portable Document Format (PDF) you will require Adobe Acrobat Reader Version 3.0 or higher, free from Adobe Systems, Inc. You can download the Acrobat Reader version that is suitable to your system from the Adobe web site

2. If you already have Adobe Acrobat Reader, Version 3.0 or higher installed, you can click here to view or print the visa application for;
