These requirements are applicable for US Citizens applying at the Washington DC Office
Please send TDS your current valid and signed passport. It must not expire within 6 months and must have at least one blank visa page. If your passport expires within 6 months, you must renew your passport and then we will secure your visa with the new passport.
Letter from sending Government Department or Agency, addressed to
Embassy of Haiti, Visa Section, Washington D.C.
The letter must explain all of the following:
Applicant's name and occupation
Purpose of the trip
Dates of the trip
Contact person in Haiti
This visa is usually valid for ninety days from date of issue, for a period of stay not to exceed 30 days.
Your application will be hand carried to the Embassy of Haiti and is issued in 1-2 business days.

These are the fees charged by the foreign government for issuing a visa. Fees are
per person per visa.
