These requirements are applicable for US Citizens applying for a Cambodia Missionary / Humanitarian visa in Washington DC

PassportPassport

Please send TDS your current valid and signed passport. U.S passport must be valid 6 months beyond intended stay and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.
A visa cannot be stamped on a page even if only one Small Stamp is on the visa page. If you do not have any blank visa pages in your passport, TDS can assist you in applying for additional pages. If your passport is not valid for 6 months beyond your intended stay, you must renew your passport and then we will secure your visa with the new passport.

It is possible to obtain a Missionary/Humanitarian visa upon arrival in Cambodia at two airports Siem Reap and Phnom Penh and certain border checkpoints between Cambodia, Thailand and Vietnam. 
Visa requirements and restrictions vary greatly when obtaining an on arrival visa. 
The local immigration officials also hold the right to refuse entry to anyone for any reason. 
We strongly recommend that you obtain your visa prior to departure. 
Should you choose to obtain your visa upon arrival, you will need to inquire with your airline if boarding is allowed without a visa in advance.

Please send a copy of your round trip airline tickets and a copy of your travel itinerary. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.

Business LetterBusiness Letter

A business letter of responsibility is required, to be typewritten on your organizations letterhead and addressed to:
Embassy of Cambodia, Visa Section, Washington DC
The letter must explain all of the following:
Nature of business to be performed
Name and address of reference to be visited
This letter should request Multiple Entries visa if required
Guarantee of return transportation and sufficient funds for the visit
The letter should be signed by an officer of the company other than the applicant.

Invitation LetterInvitation Letter

Letter of invitation on letterhead from a host organization in Cambodia. Invitation should detail intended dates of arrival and departure from the country, your name and passport number, and request any special visa conditions (Multiple Entries visa) if required. Fax or scan is accepted.

The Cambodian Missionary/Humanitarian visa is usually valid for a single entry only from date of issue for a stay of up to 30 days.

The visa is valid for entry for three months from the date of issue. If your trip requires two entries to Cambodia, you must obtain the second entry upon arrival at a border entry point or airport. Once in Cambodia you can apply to have your visa extended for one month, three months, six months or twelve months for single or multiple entries if required.

Your application will be hand carried to the Embassy of Cambodia. The visa processing time is usually not less than 10 business days. Rush service is available for a Cambodian Missionary/Humanitarian visa applications.
