These requirements are applicable for US Citizens applying for an Indonesian Tourist visa in Los Angeles

As of June 10th, 2015 the government of Indonesia will allow US Citizens enter Indonesia visa free for stays of 30 days or less.
The visa exemption can only be applied at 9 immigration checkpoints.

3. Kualanamu International Airport, Medan
4. Juanda International Airport Surabaya
5. Hang Nadim International Airport, Batam
6. Sri Bintan Pura Seaport, Tanjung Pinang
7. Batam Center Seaport, Batam
8. Sekupang seaport, Batam
9. Tanjung Uban Seaport, Tanjung Uban

This is not a Business, Work or Visitation (Social) visa. Therefore it cannot be converted to obtain other immigration permits. The maximum stays permitted for the visa on arrival is 30 days. If you plan to stay longer than 30 days you need to apply for a visa.

Single Entry visas are valid for 3 months, multiple entry visas are valid for 12 months. Your passport must be valid for 12 months from date of entry into Indonesia to be eligible for a multiple entry visa.

If you apply for an actual visa (not the Visa Free on Arrival Option)  it will be valid starting from the day it is issued. You must enter Indonesia within 90 days of issuance of the visa. You can stay in country up to 60 days.

Please send TDS your current valid and signed passport. It must not expire within 6 months for single entry or 12 months remaining validity for multiple entry visa and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.
A visa cannot be stamped on a page even if only one Small Stamp is on the visa page. If you do not have any blank visa pages in your passport, TDS can assist you in applying for additional pages. If your passport expires within 6-12 months, you must renew your passport and then we will secure your visa with the new passport.

Please send a copy of your round trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.

There are regions of Indonesia that foreigners are not allowed to visit without written permission of Indonesian authorities. The regions are Maluku, Poso, Irian Jaya, and Aceh. Those wishing to enter these regions must obtain a permit or authorization from the Department of Foreign Affairs in Jakarta. Those visiting Aceh region for work or having activities related to the Rehabilitation and Reconstruction Agency (BRR) must have authorization from BRR before applying for the visa.

The Indonesia visa is usually valid for three months from date of issue, for stay not to exceed 30 days. Extensions to 60 days are possible but must be done while in Indonesia.
The Indonesian visa must be first used within 90 days of the date of issue.
