These requirements are applicable for Non-US Citizens applying for a Madagascar Long Term Stay visa in Washington DC

Please send TDS your current valid and signed passport. It must be valid at least 6 months beyond intended stay and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.
A visa cannot be stamped on a page even if only one Small Stamp is on the visa page. If your passport is not valid 6 months beyond your intended stay, you must renew your passport and then we will secure your visa with the new passport.

Please send a copy of your round trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.
A Photocopy of Permanent Resident Card (Green Card) or valid U.S. visa and I-94 is required.
Travelers who entered the USA after April 26th 2013 should print the electronic I-94 form from the
Department of Homeland Security.
A business letter of responsibility (sample) is required, to be typewritten on your company letterhead and addressed to:
Embassy of Madagascar, Visa Section, Washington DC
The letter must explain all of the following:
Nature of business to be performed
Name and address of reference to be visited
This letter should request Multiple Entries visa if required
Guarantee of return transportation and sufficient funds for the visit
The letter should be signed by an officer of the company other than the applicant.
Letter of Recommendation from the organization sponsoring the trip, the letter must state the applicants name, purpose of the trip to Madagascar and the intended lenght of stay.
Visa is valid one month from date of issue, for stay up to 30 days. Issued to enable applicant to finalize, in Madagascar, a long term stay through the Ministry of Interior. Application should clearly specify that the applicant applies for one month visa for the purpose of obtaining a long term visa in Madagascar.
Applicant must be able to present a Police Report when applying to extend visas lenght of stay in Madagascar.

The best choice of vaccines for your trip depends on many individual factors, including your precise travel plans. Vaccines commonly recommended for travelers to Africa include those against Tetanus, Diphtheria, Polio, Typhoid, Hepatitis A, Hepatitis B, Yellow fever, Malaria Rabies and Meningitis.

Certificate is required for entry into, or travel between, some African countries. Several of these vaccines require more than one dose, or take time to become effective. It is always best to seek advice from your doctor on immunization well in advance, if possible around 6 weeks before departure.

The government of Madagascar requires proof of yellow fever vaccination upon arrival if you are traveling from a country with risk of yellow fever.
