These requirements are applicable for Non-US Citizens applying for a Mozambique Missionary / Religious visa in Washington DC

Passport

Please send TDS your current valid and signed passport. It must be valid at least 6 months beyond intended stay and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.
A visa cannot be stamped on a page even if only one Small Stamp is on the visa page. If your passport is not valid 6 months beyond your intended stay, you must renew your passport and then we will secure your visa with the new passport.

Proof of Departure

Please send a copy of your round trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.

Proof of US Status

A Photocopy of Permanent Resident Card (Green Card) or valid U.S. visa and I-94 is required.
Travelers who entered the USA after April 26th 2013 should print the electronic I-94 form from the
Department of Homeland Security.

Business Letter

A business letter of responsibility (sample) is required, to be typewritten on your company letterhead and addressed to:
Embassy of Mozambique, Visa Section, Washington DC
The letter must explain all of the following:
Nature of business to be performed
Name and address of reference to be visited
This letter should request Multiple Entries visa if required
Guarantee of return transportation and sufficient funds for the visit
The letter should be signed by an officer of the company other than the applicant.

Invitation Letter

You must obtain an official letter of invitation from your sponsor or host. The organization you will be visiting is responsible for obtaining the invitation. This letter must:
Be printed on the sponsoring organization's official letterhead
Include the sponsoring organization's contact information
Explain the applicant's relationship to the sponsoring organization
Describe the applicant's purpose and activities
Specify the length of stay
Provide accommodation details
Provide details on who will be financially responsible for the traveler

Hotel Confirmation

A copy of your confirmed hotel reservation(s). If you are staying at various hotels please provide a copy of the reservation from each hotel.

Minors

The parents of a minor visa applicant must provide a notarized parental authorization letter, signed by both parents or legal guardians along with copies of their ID

Vaccinations

Original Yellew Fever Certificate is required.
Some countries may require vaccinations if your immediately prior point of departure is from countries deemed a health risk. Please go to http://wwwn.cdc.gov/travel/contentVaccinations.aspx for the most up to date health requirements for each segment of your trip.

NOTE: The Mozambique Embassy is only open on Tuesdays and Thursdays for visa application submissions and pickups.

NOTE: All visas must be used within the first 60 days from the date of issuance if not the visa will expire and no longer be valid for entry into Mozambique. i.e. A visa issued for 6 months and multiple entries must be used within the first 60 days from the date of issue, otherwise, the visa will expire.
