Please send TDS your current valid and signed passport. Passport must be valid 6 months beyond the validity requested and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.
A visa cannot be stamped on a page even if only one Small Stamp is on the visa page. If your passport is not valid 6 months beyond the requested validity, you must renew your passport and then we will secure your visa with the new passport.

In addition to your original passport, you must also send to TDS a copy of the vital data page and any page in your passport that has a visa, entry or exit stamp on it.

A Photocopy of Permanent Resident Card (Green Card) or valid U.S. visa and I-94 is required.
Travelers who entered the USA after April 26th 2013 should print the electronic I-94 form from the
Department of Homeland Security.

Please send a copy of your round trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.

Letter from employer verifying emplotment status and leave of absence.

You will be required to upload a copy of your Employment Letter when completing the visa application.

If staying with a relative or friend who lives in Turkey a notarized Invitation Letter from the host  with their contact information, purpose and duration of your visit, conformation of where you will be staying icluding the full address, signature and date. You must provide proof of hosts status in Turkey, i.e copy of passport, residence permit and national passport details page if they are not a Turkish citizen.

You will be required to upload a copy of your Invitation Letter when completing the visa application.

A copy of the Applicant's recent bank statement or evidence of adequate finance, such as a letter from a bank, or money market statement. It is advisable to erase account numbers.

A copy of the applicant's hotel confirmation is required.
